SAX: Missing ESIS Information

Here, off the top of my head, is the main XML information missing from
SAX but available in the ESIS output of NSGMLS:

- attribute types
- public and system identifiers for notations
- notation and public and system identifiers for external data entities

There has been no agreement on whether this information belongs in
SAX, and I welcome further discussion.
