I believe that it is a binary recursive pattern:


                                     meta-data  ...
                                  /
                         meta-data
                      /           \\
             meta-data               data   ...
          /           \\ 
         /               data  ...
  context              
         \\              meta-data ...
          \\         / 
             data                   meta-data ...
                     \\          /
                         data 
                                 \\
                                    data  ...


Thus, you are right on about it being 
a "matter of context".
