Why no ']]>' in CharData?

What of that?  We are incompatible with XML documents that
use PIs, or entities, or attributes.  But SML documents would be
fully compatible with XML processors (modulo the question of
matching _content_ rather than _document_).

With these rules, < > & are the only entity
references and they are all mandatory.  Very simple.
(If we allow legacy encodings, then we need some
kind of character references as well, presumably &#xnnn; .)
